FACC December 6 General Meeting – Helping Hands Coalition, speaker

FACC’s December 6 program will be presented by Rick Chilton, with Florence’s Helping Hands Coalition. He will discuss how their organization is centering Social Determinants of Health to improve western Lane County health outcomes. He will talk about how they are helping to close acute health disparities by focusing on the conditions in which people are born, grow, live, work, worship and age. Social determinants of health, which include social, economic and environmental factors, are broadening traditional health approaches because they influence 80% of, and provide 400% better, healthy outcomes.
Rick established and is President and Program Director of the Helping Hands’ Mobile Community Social Health Center. The Center seeks to offer a pathway to live a healthier resilient life with onsite nutritious hot lunches and a mobile approach in providing clients with a personalized plan to meet all needs, including health behavior education for better lifestyle choices, and stability.
